New study aims to prevent sports-related brain injury in youngsters

Mar. 28, 2013 ? Ice hockey accounts for nearly half of all traumatic brain injuries among children and youth participating in organized sports who required a trip to an emergency department in Canada, according to a new study out of St. Michael's Hospital.

The results are part of a first-of-its-kind study led by Dr. Michael Cusimano that looked at causes of sports-related brain injuries in Canadian youth and also uncovered some prevention tactics that could be immediately implemented to make sports safer for kids.

"Unless we understand how children are getting hurt in sport, we can't develop ways to prevent these serious injuries from happening," said Dr. Cusimano, a neurosurgeon and the lead author of the study. "One would think that we know the reasons why kids are having brain injuries in sports, but until know, it was based mainly on anecdotes."

The study used data from The Canadian Hospitals Injury Reporting and Prevention Program to look at the almost 13,000 children and youth aged 5-19 who had a sports-related brain injury between 1990 and 2009. The results appeared in the journal PLOS ONE today.

The researchers categorized injuries by players' ages, what sport they occurred in and what mechanisms had caused them -- "struck by player," "struck by object" (such as net or post), "struck by sport implement" (such as ball or stick), "struck by playing surface" and "other."

Hockey accounted for 44.3 per cent of all injuries and almost 70 per cent of them occurred in children over 10 as a result of player-to-player contact or being hit into the boards.

Dr. Cusimano said they expected to see high numbers in hockey because it's Canada's "national sport."

"This shows that body contact is still an area where we need to make major inroads to preventing brain injuries," Dr. Cusimano said. "For example, enforcing existing rules and making more effective incentives and disincentives about checking from behind could make huge improvements."

Nineteen per cent of the youth who suffered brain injuries got them during soccer, with most in the 10 to 14 or 15 to 19 age group. In these age ranges, the most common cause of injury was being struck by another player, kicks to the head or head-on-head collisions. In the younger group, age five to nine, players were more likely to suffer a traumatic brain injury from striking a surface or a goal post than those in older groups.

"There's a really straightforward solution here," Dr. Cusimano said. "Padding the goal posts could have potentially prevented a large number of these brain injuries in young children."

The results also found that the youngest age group was at the highest risk for getting seriously injured in baseball. Most of the 15.3 per cent of injuries occurred in children under the age of 14, with 45 per cent of them in children under nine.

Ball and bat injuries were most common, with the majority of injuries caused because the players stood too close to the batter or bat and were not supervised by an adult.

"These results give us a very specific prevention message for kids under nine who play baseball: make helmets and supervision a mandatory," said Dr. Cusimano. "The younger the child, the more supervision they need when using things like bats and balls. Simple rules around not being close to the batter can be taught to children and adults."

Football and rugby accounted for 12.9 per cent 5.6 per cent of injuries respectively, and the majority of them were caused by tackling.

Basketball made up 11.6 per cent of injuries, mostly caused by player-to-player elbowing, which increased as players got older.

"There is a real opportunity for prevention here," Dr. Cusimano said. "Having educational programs, proper equipment, rules and other incentives that support a culture of safety in sports should be a mandate of parents, coaches, players, sports organizations, schools, sports sponsors, and other groups like governments."

Last week the federal government issued information concerning the sign-up process for Obamacare that will begin October 1 of this year.? Here are the details.

Forgetting for the moment the complexity of the application?process, I thought it might be of?interest to deal with only one aspect?the income portion of the application process that will determine whether one receives a subsidy to cover all, or a portion of the cost of the premiums, or is ineligible for any subsidy.

According to the government, when your application is submitted, the income you report will be instantaneously verified by the IRS.? There is only one small problem.? As of October, 2013, the only income data the IRS has is what was reported on your 2012 income tax return.

So, let?s say, for example, you earned?$100,000 in 2012, and this amount is reported on your 2012 return.??In this case, you?would not qualify for a subsidy.? But, let?s say,?you lost your job in 2013, and only?expect to earn?$35,000 in 2013.? In that case, you would qualify for a subsidy.? On the application you submit in October of this year, you dutifully report your 2013 income as $35,000, but, the IRS will bounce it back because its records show you make $100,000.

Of course, the converse is also true.

Then, what do you do about the pesky situation where one qualifies for a subsidy, but owes back taxes?? Is the subsidy paid, or will it be used to off-set what you owe?

Heck, why am I wasting my time on this?? The federal government is in charge.? What could possibly go wrong?

CHAPEL HILL, N.C. Researchers at the University of North Carolina at Chapel Hill have discovered that a protein found in the cells surrounding pancreatic cancers play a role in the spread of the disease to other parts of the body.

In a finding to be published in the March 25 issue of Oncogene, researchers in the lab of Carol Otey, PhD, found that the protein palladin enhances the ability of cancer-associated fibroblasts (CAFs) to assemble organelles known as invadopodia to break down the barriers between cells and create pathways for tumors to spread throughout the body. Otey is a professor in the Department of Cell Biology and Physiology and a member of UNC Lineberger Comprehensive Cancer Center.

"There's a growing body of literature that shows that these cells have a role in cancer formation and metastasis," said Otey.

Using both enzymatic action and physical force, the invadopodia create channels for tumor cells to migrate from their point of origin to other organs. Otey said that researchers, using cultured cancer cells suspended between layers of collagen, have been able to observe CAFs tunneling through the collagen layer and record cancer cells migrating through those channels.

In previous studies, researchers in the Otey lab and other labs have shown that CAFs surrounding pancreatic tumors express high levels of palladin. In healthy tissue, fibroblasts are the most common type of connective tissue found in mammals. In cancerous cells, CAFs are the most numerous cells found in the tumor microenvironment.

Researchers have begun focusing significant attention to the tumor microenvironment, as evidence grows that the cells and proteins found outside of cancer cells play a crucial role in tumor formation, growth and metastasis. Understanding the interplay between the microenvironment and the tumors could lead to new targets for treatment and screening, especially in cancers that are resistant to therapies that directly target the cancerous cells.

"Cells seem to be partnering together to form the tumor and promote its growth," said Otey.

Using pharmacological inhibitors and gene-silencing approaches, Otey and the research team discovered that disrupting palladin in CAFs reduced the ability of the cells to form invadopodia. Increasing the level of palladin in CAFs, by contrast, increased the rate of growth and metastasis of tumors in mouse models. Their results indicate that palladin may be part of a molecular pathway that includes two additional molecules, protein kinase C and Cdc42.

"These results demonstrate that the behavior of CAFs plays a very important role in modulating the behavior of tumor cells, and also point to a specific molecular pathway that could be a useful drug target for inhibiting tumor progression," said Otey. Since Otey discovered palladin more than a decade ago, researchers in her lab have examined the protein's role in both healthy and cancerous cells. Citing her own work, research from the Brentnall lab at University of Washington, and corroborating work such as a study from the Cukierman lab at Temple University's Fox Chase Cancer Center that found high levels of the palladin protein correlated strongly with low survival rates in renal carcinoma patients, Otey said that the evidence points toward a strong correlation between palladin expression in CAFs and the aggressiveness of tumor progression.

In future research, Otey plans to examine the levels of palladin in other types of cancer. As the Fox Chase study suggests, the mechanisms that she and her collaborators have discovered may play a role in cancers other than pancreatic.

"Knowing more about this may give us better tools to slow down metastasis," said Otey.

High court weighs drug companies' generics policy

WASHINGTON (AP) ? Federal regulators are pressing the Supreme Court to stop big pharmaceutical corporations from paying generic drug competitors to delay releasing their cheaper versions of brand-name drugs. They argue these deals deny American consumers, usually for years, steep price declines that can top 90 percent.

The Obama administration, backed by consumer groups and the American Medical Association, says these so-called "pay for delay" deals profit the drug companies but harm consumers by adding 3.5 billion annually to their drug bills.

But the pharmaceutical companies counter that they need to preserve longer the billions of dollars in revenue from their patented products in order to recover the billions they spend developing new drugs. And both the large companies and the generic makers say the marketing of generics often is hastened by these deals.

The justices will hear the argument Monday.

Such pay-for-delay deals arise when generic companies file a challenge at the Food and Drug Administration to the patents that give brand-name drugs a 20-year monopoly. The generic drugmakers aim to prove the patent is flawed or otherwise invalid, so they can launch a generic version well before the patent ends.

Brand-name drugmakers then usually sue the generic companies, which sets up what could be years of expensive litigation. When the two sides aren't certain who will win, they often reach a compromise deal that allows the generic company to sell its cheaper copycat drug in a few years ? but years before the drug's patent would expire. Often, that settlement comes with a sizable payment from the brand-name company to the generic drugmaker.

Numerous brand-name and generic drugmakers and their respective trade groups say the settlements protect their interests but also benefit consumers by bringing inexpensive copycat medicines to market years earlier than they would arrive in any case generic drugmakers took to trial and lost. But federal officials counter that such deals add billions to the drug bills of American patients and taxpayers, compared with what would happen if the generic companies won the lawsuits and could begin marketing right away.

A study by RBC Capital Markets Corp. of 371 cases during 2000-09 found brand-name companies won 89 at trial compared to 82 won by generic drugmakers. Another 175 ended in settlement deals, and 25 were dropped.

Generic drugs account for about 80 percent of all American prescriptions for medicines and vaccines, but a far smaller percentage of the $325 billion spent by U.S. consumers on drugs each year. Generics saved American patients, taxpayers and the healthcare system an estimated $193 billion in 2011 alone, according to health data firm IMS Health.

But government officials believe the number of potentially anticompetitive patent settlements is increasing. Pay-for-delay deals increased from 28 to 40 in just the last two fiscal years and the deals in fiscal 2012 covered 31 brand-name pharmaceuticals, Federal Trade Commission officials said. Those had combined annual U.S. sales of more than $8.3 billion.

The Obama administration argues the agreements are illegal if they're based solely on keeping the generic drug off the market. Solicitor General Donald Verrilli, speaking at Georgetown Law School recently, noted that once a generic drug gets on the market and competes with a brand-name drug, "the price drops 85 percent." That quickly decimates sales of the brand-name medicine.

"These agreements should actually be considered presumptively unlawful because of the potential effects on consumers," Verrilli said.

In the case before the court, Brussels, Belgium-based Solvay ? now part of a new company called AbbVie Inc. ? reached a deal with generic drugmaker Watson Pharmaceuticals allowing it to launch a cheaper version of Solvay's male hormone drug AndroGel in August 2015. Solvay agreed to pay Watson, now called Actavis Inc., an estimated $19 million-$30 million annually, government officials said. The patent runs until August 2020. Watson agreed to also help sell the brand-name version, AndroGel.

Actavis spokesman David Belian disputed the government's characterization of the agreement with Solvay. Belian said that in addition to licensing agreement over Solvay's Androgel patents, Watson was being compensated for using its sales force to promote AndroGel to doctors.

AndroGel, which brought in $1.2 billion last year for AbbVie, is a gel applied to the skin daily to treat low testosterone in men. Low testosterone can affect sex drive, energy level, mood, muscle mass and bone strength.

The FTC called the deal anticompetitive and sued Actavis.

The 11th U.S. Circuit Court of Appeals in Atlanta rejected the government's objections, and the FTC appealed to the Supreme Court.

The federal district and appellate courts both ruled against the government, AbbVie, which is based in North Chicago, Ill., said. "We are confident that these decisions will be upheld by the Supreme Court."

The Generic Pharmaceutical Association's head, Ralph Neas, said the settlements are "pro-consumer, pro-competition and transparent." He said every patent settlement to date has brought a generic drug to market before the relevant patent ended, with two-thirds of the new generic drugs launched in 2010 and 2011 hitting the market early due to a settlement.

"By doing what the FTC wants, you're going to hurt consumers rather than help them," said Paul Bisaro, CEO of Actavis of Parsippany, N.J.

Bisaro said consumers will save an estimated $50 billion just from patent settlements involving Lipitor, the cholesterol-lowering drug made by Pfizer Inc. of New York that reigned for nearly a decade as the world's top-selling drug.

Lipitor's patent ran until 2017, but multiple generic companies challenged it. Pfizer reached a settlement that enabled Actavis and a second company to sell slightly cheaper generic versions starting Nov. 30, 2011, and several other generic drugmakers to begin selling generic Lipitor six months later. The price then plummeted from Pfizer's $375 to $530 for a three-month supply, depending on dosage, to $20 to $40 for generic versions.

Because generic companies tend to challenge patents of every successful drug, the FTC's position would impose onerous legal costs on brand-name drugmakers and limit their ability to fund expensive research to create new drugs, said the Pharmaceutical Research and Manufacturers of America, which represents brand-name drugmakers.

According to the 2010 RBC Capital Markets study, when trial victories, settlements between drugmakers and dropped cases are combined, generic companies were able to bring their product to market before the brand-name drug's patent expired in 76 percent of the 371 drug patent suits decided from 2000 through 2009.

Consumer, doctor and drugstore groups have lined up to support the Obama administration in this case.

"AARP believes it is in the interest of those fifty and older, and indeed the public at large, to hasten the entry of generic prescription drugs to the marketplace," said Ken Zeller, senior attorney with the AARP Foundation Litigation. "Pay-for-delay agreements such as those at issue in this case frustrate that public interest."

The American Medical Association, the giant doctors' group, believes pay-for-delay agreements undermine the balance between spurring innovation through patents and fostering competition through generics, AMA President Dr. Jeremy A. Lazarus said. "Pay for delay must stop to ensure the most cost-effective treatment options are available to patients."

Drugstores also believe pay-for-delay deals "pose considerable harm to patients because they postpone the availability of generic drugs which limits patient access to generic medications," said Chrissy Kopple of the National Association of Chain Drug Stores.

Eight justices will decide this case later this year. Justice Samuel Alito did not take part in considering whether to take this case and is not expected to take part in arguments.

THURSDAY, March 21 (HealthDay News) ? Only 11 percent of the estimated 79 million Americans who are at risk for diabetes know they are at risk, federal health officials reported Thursday.

The condition, known as prediabetes, describes higher-than-normal blood sugar levels that put people in danger of developing diabetes, according to the U.S. Centers for Disease Control and Prevention.

?We have a huge issue with the small number of people who know they have it. It?s up a bit from when we measured it last, but it?s still abysmally low,? said report author Ann Albright, director of the CDC?s Division of Diabetes Translation.

?We need people to understand their risk and take action if they are at risk for diabetes,? Albright said. ?We know how to prevent type 2 diabetes, or at least delay it, so there are things people can do, but the first step is knowing what your risk is ? to know if you have prediabetes.?

Things that put people at risk for prediabetes include being overweight or obese, being physically inactive and not eating a healthy diet, Albright said. These people should see their doctor and have their blood sugar levels checked, she said.

There is also a genetic component, Albright said, which is why having a family history of diabetes is another risk factor. ?Your genetics loads the gun, then your lifestyle pulls the trigger,? she said.

According to the report, published in the March 22 issue of the Morbidity and Mortality Weekly Report, the lack of awareness of prediabetes was the same across the board, regardless of income, education, health insurance or access to health care.

One expert found the numbers troubling.

?People don?t know about prediabetes, they don?t exercise, they don?t eat appropriate foods and we are going to have many more diabetics in the near future than we have now,? said Dr. Spyros Mezitis, an endocrinologist at Lenox Hill Hospital in New York City.

The danger of prediabetes is that it can progress to full-blown diabetes, with all the complications that condition entails, including heart, kidney, circulation and vision problems.

Albright noted that 30 percent or more of those with prediabetes will develop diabetes over the course of a decade.

The number of Americans with diabetes is already staggering. According to the American Diabetes Association, 25.8 million children and adults in the United States ? 8.3 percent of the population ? have diabetes.

?The good news is we know there are things you can do to prevent or delay the development of type 2 diabetes,? Albright said. ?You can prevent or delay diabetes if you lose 5 percent to 7 percent of your body weight and get 150 minutes of physical activity a week.?

Another expert said it starts with what you eat.

Women are a major driving force of photography. No sooner had French pioneer Louis Daguerre introduced the media to the public in 1839 than it began to be used as a method to portray female beauty. It was American chemist John W. Draper who ushered women into photography with a portrait of his sister Dorothy. And ever since, the suspicion has been that a wooing Orpheus lurks behind the cold technology of the camera - an artist whose intentions are to captivate and seduce women with photographs.?

But women have not only left their mark on photography as models and backdrops. They have contributed stylistically behind the camera itself as well. On the occasion of International Women's Day on 8 March 2013, the exhibition "Women" explores the feminine side of photography. Fifteen photographic approaches document aesthetic and temporal characteristics in images and depictions of femininity.

The exhibition begins with the complex light montages from the pre-war work of Heinz Hajek-Halke. The Berlin-based photographer exemplifies the staged embodiment of femininity during the avant-garde movement. Hajek-Halke?s experimental techniques did not simply display women's bodies, the photographer rendered pygmalionesque recreations of light on silver gelatin paper.?

The approach of many photojournalists in the postwar era -Robert Lebeck, Stefan Moses, Hannes Kilian and Max Scheler ? was quite different. Their names stand for a new international image of the German woman. Portraits of American divas such as Eartha Kitt and Elizabeth Taylor document the return of glamor to the Adenauer Republic. But women at work also garnered the attention of this generation of photographers. Among others, Robert Lebeck?s 1964 photograph of Sicilian washerwomen in Cullera exemplifies journalistic art, and ties in with so-called "life photography". The dancing workers in this photo illustrate far more than playful flirtation with the camera ? the photo represents a photographic search for dignity and grace.

Scientists from Hong Kong offer a robust, solid and viable molecular tools to identify deer DNA even in highly processed products

Many Europeans are fretting these days over what they eat, and whether horse meat might have adulterated their pork chops. Food fraud has been dominating headlines globally - calling for new policies in law enforcement and more robust methods for successful food identification and authentication. As companies and manufacturers resort to fraudulent practices to extract more cash from the gullible public, it is estimated that up to 7% of the consumer supply chain contains hidden ingredients (i.e. not disclosed on the label). And while all too often policymakers seem oblivious to the problem, the growing awareness of plain criminal activity in food supply has stimulated an increase in published research on animal DNA testing, either for the identification of species or for the genetic linkage of a sample to a particular organism.

The conventional methodologies employed for the determination of species origin in meat products have predominantly applied molecular methods of immunochemical, electrophoretic and chromatographic analysis of proteins. For those cases where reliance on morphological characteristics is impractical or impossible, scientists offer now novel techniques allowing the identification of species specific DNA sequences. Among these is a technique that relies on the much debated DNA barcoding - developed by researchers from the Government Laboratory in Hong Kong who have come up with a method that permits DNA detection of the fraudulent substitution of commercial deer products, regardless of their physical state, so that identification by morphology (form) is not required.

Deer meat has come a long way as an alternative to pork and beef. But it has continued to catch up with consumers steadily if slowly over the last decade, mainly due to its nutritive and therapeutic values but also versatile serving methods. And while venison is low in fat and high in protein, iron, zinc, selenium, vitamin E and omega-3 fatty acids - adding up to one healthy meal in view of recent scams, it has become vital to provide tenable methods of effective deer meat verification.

The article published recently in DNA Barcodes (http://www.versita.com/dnabra), an open access journal by Versita, describes the protocol set up by Dr. W.M. Sin and Dr. Y.K. Tam - to examine whether DNA methods alone suffice to detect fraudulent substitution of commercial deer products or, whether any additional protocols are necessary to detect fraudulent substitution of cattle and water buffalo tendons (HK$50-80) for deer tendons (HK$280-640). The research confirmed that no other method proves as efficient and straightforward as the use of DNA barcodes, which are sufficient on their own to detect such substitution for deer in all tendon products, except for glue. Furthermore, the research findings permit DNA detection of fraudulent substitution of commercial deer products, regardless of their physical condition.

The attractiveness of this method lies in its utility. Commenting on the research, Prof. Jan Pawlowski, from the Department of Genetics & Evolution at University of Geneva, Switzerland, says: "The authors did an excellent work, offering a robust, solid and viable molecular tools to identify deer DNA even in highly processed products. This is a new example showing the importance of DNA barcoding for traceability of commercial products".

The method may well be embraced by law enforcement authorities and forensic scientists as an inexpensive alternative that only requires standard laboratory techniques for handling DNA. The move helps to combat the widespread mislabeling of deer, which results in cheaper meat being sold as a more expensive deer variety. It also opens a prospect for more in-depth research into other food supplies, and the roll-out of new technology that would allow a systematic use of barcoding. With the new food scandals unraveling on a daily basis, DNA barcodes have a great potential to prevent and combat wildlife crime.

Buffalo Bros Pizza, Wings & Subs is a self-described ?TCU neighborhood hangout? that has been catering to TCU students, faculty and athletics teams for five years.?

Since it opened its doors in November 2007, Buffalo Bros has represented a slice of Buffalo, New York for the local community, owner Ed McOwen said.

McOwen grew up in Buffalo, where he attended culinary school and met his wife, Shannon.?After the two started a family, and with opportunities to start a successful business dwindling in the Northeast, they decided to move to Shannon?s hometown of Fort Worth.?

?It kind of started out with the idea of, ?Let?s bring a little bit of home, here [to Fort Worth],?? McOwen said.?

McOwen grew up as a big fan of college sports and, after moving to Fort Worth, adopted the Horned Frogs as his favorite team. While Buffalo Bros started out as a Buffalo sports-centric bar, working with TCU Athletics was a ?natural connection? as the business grew, McOwen said.?

The relationship began as the establishment advertised with TCU basketball and baseball a year after opening. It was shortly thereafter when TCU Athletics came to Buffalo Bros with the idea of hosting a men?s basketball show featuring then-head coach Jim Christian. It established a connection with TCU sports that was unique to the area.?

?It adds to the whole idea of being a TCU sports bar,? McOwen said.?

A year after the Coaches Show began at Buffalo Bros, the establishment had cultivated a professional relationship with TCU. The sports bar has started to promote the tennis team this year and has also been catering tailgating events at baseball games for the past three years.

Kristi Setzer-Mazzola, general manager of Buffalo Bros for over two years, called the relationship between the restaurant and the school ?symbiotic? and that they have gladly supported events like the Recreation Center?s 10-year anniversary.?

McOwen said: ?It?s never been about the money or anything like that.?[While] it?s partly about getting our name out, it?s also about supporting the programs ... Hopefully, people make the connection - ?They support the school, and they support that team - let?s go support them.???

With Buffalo Bros having further ingrained themselves in TCU school culture, even the national media took notice - in September 2012, Yahoo! Sports named Buffalo Bros as one of the best college sports bars in the country. For Setzer-Mazzola, it is a further indication of the connection Buffalo Bros is making with TCU.

?To me, it?s more than being a convenient location ... it?s a destination,? she said. ?This is my bar - that?s what I very much enjoyed at a bigger school, at college bars ... that?s what I want TCU to be. A home away from home,? she said.

As Buffalo Bros continues its service to the local community and the athletic program, McOwen envisions Buffalo Bros being involved with all TCU sports.?

?I?d like for everyone supporting the students to feel free to call and ask for anything,? McOwen said.?

The school shares similar sentiments.

"TCU is very proud of its relationship with Buffalo Bros,? said Mark Cohen, assistant athletics director, in a recent statement. ?In addition to serving as host of the Trent Johnson Radio Show and providing the pizza at home tennis matches, Buffalo Bros has truly become synonymous with the Horned Frogs."

For McOwen, Buffalo Bros' relationship with TCU athletics goes beyond the school, and believes it affects the surrounding community.

?Being a part of the program turns around and makes us more and more a part of the community,? McOwen added. ?Not just talking about [having a relationship with TCU], but actually doing it.?

Justin Bieber is recovering after fainting backstage at a concert in London.

A spokeswoman for Bieber said late Thursday that the 19-year-old pop star was given oxygen and took a 20-minute reprieve after fainting backstage at London's O2 Arena.

Bieber posted a shirtless photo of himself in a hospital bed late Thursday with the caption: "Gettin better listening to Janis Joplin." Before that on Twitter he thanked "everyone pulling me thru tonight."

"Best fans in the world," he wrote. "Figuring out what happened. Thanks for the love."

Jazz Chappell, a 20-year-old concertgoer who brought her younger sister and her friend to the show, said Bieber announced he couldn't breathe and needed water. He resumed performing, but then had to be helped offstage by a backup dancer. In the nearly 30 minutes he was offstage, some fans started to leave. Once his manager announced what had happened, Chappell said many fans in the audience were gasping and crying, while others kept cheering for him to return.

"I thought, 'Give the guy a break. He just fainted. He's not a performing horse. Let him rest a second,'" said Chappell.

In a video of the concert posted online, Bieber's manager, Scooter Braun, appeared onstage and told the crowd that the singer was feeling "very low of breath" but would come back to finish the show.

In another video posted online that claims it is from Thursday's show, Bieber appears out of breath during a performance of his uptempo hit, "Beauty and a Beat." He begins to fade as he is dancing and puts his hand on his forehead and bends over to catch his breath.

Chappell said Bieber, who's in London to perform four concerts at the O2, later returned and performed low-energy renditions of his hits "Boyfriend" and "Baby."

Braun later tweeted "everyone please give him a little space and he will be ok. Im sure he appreciates the support ... Tough kid proud night once again he always finishes the show. Full out. True pro..."

The incident caps a difficult week for Bieber. He was forced to apologize to outraged fans who accused him of taking the stage almost two hours late for his first concert at the O2 on Monday. He insisted he was only 40 minutes late and blamed "technical issues." He took to Twitter to vent his frustrations with the media's portrayal of the incident.

Bieber's Friday night concert at the O2 remained scheduled as of early Friday morning.
